If sending a person to the hospital extends the person's life, for return to the community, or even just signals the person may die, then natural selection factors change for selecting natural mutations which can occur inside the hospital as well as outside it. The hospital changes the opportunities for spread and mutations that will get back out into the community and remove those that are more certain to lead to death.

Darwin's book "On the Origin of the Species" was regarded as one of the great reads of the 1800s, and is.

But I was never able to read it (despite four tries at various times in my life). I bought the Audible version and found out my problem was that he wrote the book in a deep British accent. Having a Brit with a deep accent read it to you makes all the difference. It is indeed a great book and Darwin was crystal clear and completely convincing. I personally have no ability whatsoever in 'dong a British accent' which is why I couldn't even read one or two pages of his book before.
